Resource 3: Algorithm for Fall Risk Screening and Interventions

The Algorithm for Fall Risk Screening and Interventions is based on global guidelines and emphasizes opportunistic screening for individuals aged 65 and older. It begins with a set of preliminary questions assessing fall history, unsteadiness, and fear of falling. Depending on the responses, individuals may be categorized into high, intermediate, or low fall risk categories.

High-risk individuals undergo thorough medical reviews, including assessments of medication, orthostatic hypotension, and concerns about falling. Referrals for therapy, vision checks, and osteoporosis screenings are recommended. Interventions such as physical therapy and home safety modifications are also advised.

Intermediate-risk individuals are encouraged to engage in fall prevention programs and are reassessed annually.

Low-risk patients receive fall prevention education and are encouraged to follow movement guidelines, with annual reassessments.

This algorithm is designed to be used by primary health care providers. However, the Opportunistic Screening can be used by anyone to help indeitfy older adults who are at risk for falling. It's good to be aware of this tool, what contirubutes towards someone being at risk for falling, and what interventions are available for these individuals.
