Patients at High Risk for Falling

Patients at High Risk for falling are patients who have recently experienced a fall and are at risk of experiencing another fall.

There are three items that need to be considered for patients at a High Risk for falling:

  • Multifactorial Falls Risk Assessment
  • High Risk Interventions
  • High Risk Follow Up

This type of prevention is tertiary prevention, a fall has already happened and the goal is to prevent another fall. Patients who have already experienced a fall are at great risk for falling again, and having a more serious outcomes. This is the time to prevent future hip and knee replacements, and increase patients' mortality and morbidity.

This is a great opportunity for primary health care providers to be advocates for their patients and coordinate the appropriate interventions across different health care disciplines.

These patients should be followed up with much more frequently than others, at least every 30-90 days. It is very likely that without appropriate interventions and adequate follow up the patient will experience a serious fall.
